Job Summary:
As one of the care managers on the team, you will work alongside practitioners to provide services to families who are experiencing challenges in the family unit, specifically those which impact safety and connection. You will work with the families to build relationships-in the family unit and within the community-that increase their resources and provide linkages to services.
What Will You Do:
+ Provide concrete support to families that are receiving services
+ Identify gaps and barriers to services and advocating for families
+ Provide on-going assessments of families by engaging and partnering with the parents throughout the course of program participation
+ Refer families to services in a timely manner, prioritizing those based on severity of needs
+ Ensure the successful linkages to community resources by providing transportation
+ Develop and maintain relationships with a diverse array of community partners
+ Collaborate with the team of practitioners and supervisors to identify immediate service needs and resources for families
+ Participate in advisory councils/boards within their local community/area of service to gather beneficial information to assist in supporting families
What We Need to See:
+ Bachelor's degree in Psychology, Social Work, or equivalent experience within the child welfare system
+ 1+ years of experience in parent/family support, maternal-child health
+ Valid driver's license and must be insurable under the agency's automobile insurance policy.
