STATUS:Part-time, Non-Exempt (30 hours a week)
HOURLY RATE: up to $22.50 per hour
SUMMARY
The Finger Lakes Caregiver Institute (FLCI) is a specialized program at Lifespan serving caregivers of older adults. FLCI Care Managers focus on serving family/friend caregivers of individuals with memory loss or forms of dementia. FLCI Care Managers help caregivers by providing unbiased information about care options and linkage to services for their care partner as well as support, education and programs for themselves as a caregiver. Care Managers offer a holistic assessment of the caregiver’s and the person living with memory loss’ needs by identifying health, financial, emotional, functional and environmental concerns. Care Managers offer care options, solutions and guidance navigating challenging systems. Care Managers may facilitate support groups and assist with caregiver education events.Services are provided in the 10 County region of Monroe, Chemung, Genesee, Livingston, Ontario, Seneca, Steuben, Schuyler, Wayne and Yates counties. This position additionally plans, implements and oversees Memory Café programs in Genesee Co. Conducts outreach and education in Genesee County.
DUTIES AND RESPONSIBILITIES
1.0 Provide care management to caregivers and their care partners.
+ Accept referrals from various sources including, physician practices, NY Connects, Area Agencies on Aging, self-referrals, etc.
+ Complete comprehensive assessments to determine caregiver goals.
+ Create and implement care plans that meet caregiver needs and preferences.
+ Reassess goals and modifies care plans as necessary.
+ May provide information and referral, advocacy, benefit and entitlement counseling.
+ May complete or assist with Medicaid or other benefit applications.
+ Connect caregivers and clients to the Alzheimer’s Association.
+ Complete all documentation within required timeframes.
2.0 Arrange mental health interventions for caregivers and their care partners when necessary.
+ Conduct mental health screenings when appropriate.
+ Provide supportive counseling to caregivers and care partners.
+ Work together with the physician office care team to provide linkage to appropriate ongoing mental health community services.
3.0 Café Coordination. (Genesee County only. 4/year max)
+ Plan all aspects of Memory café sessions.
+ Identify locations and make all arrangements with location staff.
+ Plan agenda for Café sessions, including food, activities and entertainment if applicable.
+ Complete contracts with all sites and outside vendors.
+ Register all Memory café participants, including screening for eligibility.
+ Oversee and ensure completion of all event surveys.
4.0 Program
+ Run Memory Café sessions on site at all locations on open days.
+ Complete intake paperwork for all participants.
+ Schedule Cafes and ensure coverage on site on open days/times.
+ Plan and organize activities for all Memory Cafe open days.
+ Ensure Café sites are a safe and welcoming environment for all caregivers, care receivers and volunteers.
5.0 Outreach
+ With team, arrange and conduct community presentations for visibility of Memory Café and drop-in respite programs and recruitment of volunteers.
+ With team, conduct outreach to community groups, faith communities, and volunteer organizations.
+ With team, conduct marketing to advertise all Memory café sessions to recruit participants.
6.0 As a member of the inter-agency team.
+ Participate in all FLCI staff meetings and training.
+ Participate in all Lifespan staff meetings and trainings.
+ Participate in Quality Assurance activities.
+ Accept and seek guidance and clinical supervision.
+ Collaborate with Lifespan team members and professional partners.
7.0 Timely Completion of all required documentation and data entry into appropriate database.
8.0 Maintain up to date professional knowledge of the geriatric field, with special focus on Alzheimer’s
Disease and Dementia, care options, community services, entitlement benefits, clinical skills and competencies through reading, workshops and conferences.
9.0 Is aware of and actively supports the Agency’s initiative of being collaborative and community focused, innovative. Understands, demonstrates and values the ideals of diversity, equity and inclusion in all aspects of their position.
10.0 Other duties as assigned by supervisor.
Requirements
QUALIFICATIONS
EDUCATION:Bachelor of Social Work or equivalent with experience.
EXPERIENCE:One year in social work, with at least one year of experience working with caregivers and persons with dementia, geriatrics or comparable human services experience is preferred. Experienced with competing Medicaid applications a plus.Bilingual Spanish a plus.
PHYSICAL AND MENTAL REQUIREMENTS
· Must be able to meet with clients in their homes or the greater community.
· A portion of the work is performed at a desk or at a personal computer station.
· Verbal, written and telephone communication skills are required.
· Strong written and computer skills needed.
· Interface with all levels of personnel.
· Ability to maintain composure under strict deadlines and difficult situations.
· Ability to work in multi-cultural situations.
· Access to reliable transportation.
· Valid NYS Driver’s license
