Department:1006090 POPULATION CARE MANAGEMENT - EH SS

Job Description:Assists the RN Care Coordinator in the follow up regarding information in the care plan, service auths, and the submission of data in to the multiple different health plan sites along with the Department of Health and Human Services sites. As well as assists in the reinforcement of the goals, completed by the RN Care Coordinator, and the collaboration of care with the care team for that member.

Provides communication with patients, care teams, and other stakeholders to ensure that all relevant information is available to provide the best possible quality of careAssists the RN Care Coordinator with scheduling appointments, coordinating care, providing follow-up services to patients, submits data to health plans, and any other duties as assignedTracks and maintains patient records and insurance informationProvides administrative support including data entry, documentation in the Electronic Medical Record, and other duties as requiredCollaborates with other departments to ensure the efficient delivery of care while demonstrating a strong commitment to providing compassionate and quality care

This position will work the following:

64 hours per pay periodDay Shift: 8:00AM - 4:30PMNo weekends or Holiday'sThis is a hybrid position at the Virginia Clinic

Licensure/Certification Qualifications:Graduate of LPN, Medical Secretary, Health Unit Coordinator, Medical Assistant, Paramedic, EMT or similar programMinimum of 2 years’ experience in a medical setting if no education meets the Education Requirements listed
