Care Coordinator
Position OverviewPosition Summary:
United Health Services is seeking a Care Coordinator to support patients through all stages of their care journey. This role is responsible for coordinating services, managing referrals, and ensuring seamless communication between patients, providers, and the healthcare team. The Care Coordinator assists in developing care plans, tracks progress, and helps remove barriers to care, promoting positive patient outcomes.

The ideal candidate will be detail-oriented, compassionate, and highly organized, with a strong commitment to patient advocacy. Primary Work Shift:

Day

Compensation Range:

$25.27 - $37.90 per hour, depending on experience

You will be eligible for benefits if you are hired into a regular position with at least 24 scheduled weekly hours.

Job Responsibilities

Identifies PCMH high risk caseload.

Outreaches and engages with UHS System patients for care coordination/care management.

Assists in care coordination efforts as a member of the care team.

Documents efforts in electronic medical record to include but not limited to NextGen, and Allscripts.

Develops and implements care plans as a member of the care team.

Attends care conferences as necessary. Also, may organize and/or lead care conferences as necessary.

Position RequirementsMinimum Required:

Associate’s Degree in a Human Service-related field

May accept 5+ years of related care coordination experience in lieu of an 
Associate's Degree..

Preferred:

Bachelor’s Degree in Science or Arts in Human Services related field
