About the Role: As a Care Coordinator, you will be responsible for managing day-to-day care staffing for clients in both our In-Home (Kansas) and HCY (Missouri) programs. You’ll work closely with families, caregivers, and clinical teams to ensure quality care, timely communication, and continuity of services.
