As a Care Coordinator for our WISe Program in Pierce County, you’ll play a vital role in supporting children and families on their path to stability and well-being. This role is about meeting families where they are—both emotionally and physically. You’ll travel throughout Pierce County to connect with clients in their homes, schools, or community settings, ensuring they receive the compassionate, individualized care they deserve. Flexibility is essential, as this role requires availability to work some evenings, typically 2-3 per week, to support family Child and Family Team (CFT) meetings. Additionally, you will participate in a rotating schedule to hold the 24/7 WISe on-call crisis line, offering vital phone support for one week at a time. Acting as a trusted guide and advocate, you will collaborate with a multidisciplinary team to address challenges holistically, ensuring every family has the tools, resources, and unwavering support to overcome challenges. Your presence will be a steady source of reassurance, helping families build resilience, restore hope, and move toward a brighter future.

RESPONSABILITIES
Build, plan, facilitate and communicate Child and Family Team (CFT) meeting details with WISe team members, clients and families Maintain a caseload of 10 -15 clients Ensure to incorporate the input and needs of the youth when forming the team Establish and sustain an influential team culture by inviting CFT members to propose, discuss, and accept ground rules for working together Over time, work to transition facilitation to allow for youth and/or family members to be facilitators of their meetings Cultivate and maintain relationships with community members and providers Utilize consensus-building techniques to meet the needs of the youth and family Provide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriate Maintain team focus on the scope of work for the progress/movement toward transition, acknowledging and celebrate successes and changes Stay abreast of all up-to-date Cross System Care Plans for each client and any other related documentation Willing to respond to crises and be part of a rotating on-call systemQUALIFICATIONS
REQUIRED
Bachelor’s degree OR Associate's degree or equivalent + 1 year of applicable work experience OR High school diploma/GED + 2 years of applicable work experience working with youth or families or working in the social service field. Must be 21 years of age or older Available to support in the WISe 24/7 on-call rotation crisis line In person work and required travel throughout Pierce County Flexibility to work evenings two to three times per week, and weekends Clearance of TB test, fingerprints, and any other state or federal requirements Own vehicle; valid Washington Driver’s License and insurability by Seneca's automobile insurance carrierPREFERRED
Experience in WISe or other WRAP programs Bilingual skillsSKILLS OF AN IDEAL CANDIDATE

Full-time 10:00 am- 6:00 pm Available to support in the WISe 24/7 on-call rotation crisis line (schedule ahead) Mon-Fri. Flexibility to work evenings two to three times per week
