Care Connections Master of Social Work
Job Summary Care Connections MSW The Care Connections MSW is responsible for assessing and implementing social work services in the outpatient setting. Assists population health team with difficult and complex social situations and provides social work interventions as appropriate. The Care Connections MSW will provide education and contact information to patients and families regarding needed community resources. Provides psychosocial assessments and interventions to improve or maintain the social, emotional, and physical health status of the patient. This person works collaboratively with physicians, mid-level providers, staff and other health care professionals to provide a medical home and care coordination function across the health care continuum focusing on enhancing the patient experience and supporting the medical home. Is an integral member of the health care team who works to ensure safety, best practice and high quality standards of care are maintained across the continuum. Responsibilities include assisting the medical office staff in developing and coordinating a wide range of self-management support and chronic-care management activities for the clinic's affected patient population. Focus will be on Care Connections applicable to the assigned medical home offices. Success will be measured by the results of the process and outcome performance measures of the population of patients in the clinic. This position supports the Patient Centered Medical Home Concept by working closely with the clinic and Care Connections Team members navigators to develop systems and promote effective self-management of chronic-care and assist patients with navigating the healthcare system. Qualifications + Education: Graduate Degree, required. Graduate of a school of social work that is approved by the Council on Social Work Education + Experience: Minimum of two years professional social work experience in a healthcare, hospital, or community health setting providing clinical and counseling services is required. + Licensure/Certification: + LCSW, if regulations demand is required. + LCSW or eligible for licensure (LCSW-A). + Additional Skills (required): Ability to successfully complete generic and department specific skills validation and competency testing; knowledge of chronic disease issues; knowledge of community resources; knowledge of department processes and needs. Demonstrate above average ability in the following areas: managing a complex and varied work load, understanding physical and psychosocial assessment parameters.
