The Care Coach evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.
The Care Coach:
Ensures a positive and healthy prenatal and postpartum experience by providing comprehensive support, education, and resources to expecting mothersIdentifies and resolves barriers that hinder effective careEnsures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning.Understands own work area professional concepts/standards, regulations, strategies and operating standards.Makes decisions regarding own work approach/priorities, and follows direction.Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.Quarterly travel to Humana corporate office in Louisville, KyUse your skills to make an impact
Required Qualifications
Must reside in the state of Kentucky or in a county that is contiguous to the Kentucky state border in Ohio, Indiana, Illinois, Missouri, Tennessee, Virginia, or West Virginia.Associates Degree1+ year of related maternal-child health experienceKnowledge of Microsoft Office Word and ExcelExceptional verbal/written communication and interpersonal skillsAbility to use a variety of electronic information applications/software programsPreferred Qualifications
