Job Title: Bereavement Support Group Facilitator
Department: Social Work                     
Location: Rochester Regional Health Home Care - Monroe Ave                          
Hours Per Week: Per Diem / Remote     
Schedule: Monday - Friday, Days | 1-2 hours per session                                

SUMMARY:
This position facilitates support groups that are attended by hospice family members as well as the community at large.  The Facilitator develops bereavement material to disseminate as needed and educate the Bereavement and/or assigned hospice staff in understanding and dealing with clients, family and community needs.

RESPONSIBILITIES:

Conducts bereavement group sessions with support groups, family members and staff to assist them in understanding, dealing with/adjusting to conditions and supporting the clients/patients.

Evaluates needs of bereaved family members and referrals from wider community.

Identifies and creates bereavement material for educational purposes.

Organizes and conducts bereavement support groups, based on perceived needs or as requested by Manager of Bereavement Services. 

Interprets grief responses and understands psychosocial factors in order to assess client needs. 

Determines and refers appropriate group participants for individual counseling or other social supports.

Documents each group process summary and identifies situations or people needing follow-up.

REQUIRED QUALIFICATIONS:

Bachelor of Science degree in human services; Master’s degree preferred in related field such as social work, counseling, psychology or family therapy.

A minimum of three years of group facilitation experience; demonstrated understanding of supportive counseling. 

Expertise in training/development or adult education to include needs assessment, instructional design, presentation/public speaking, a variety of delivery methodologies, facilitation, coaching and learning transfer evaluation.

Demonstrates knowledge of the grief and bereavement field, as well as, group dynamics and community resources.

Excellent interpersonal, leadership and organizational skills.

Effective written and verbal communication. 

Proficient in Microsoft Office, PowerPoint and other related software packages.

Must have ability to travel.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
