Purpose:
The purpose of this job is to be part of a collaborative care team that provides behavioral health assessments and treatment to service patients who have a behavioral health diagnosis. This level of service can be an outpatient treatment program or equivalent to outpatient

Responsibilities:

Evidence-Based Treatment: Utilizes relevant and evidence-based treatment approaches as appropriate. Patient Assessments: Conducts assessments of new patients, documents clinical decisions, and adapts treatment models for service delivery. Changing Patient Needs: Assesses and incorporates changing patient needs into treatment plans for various programs (outpatient, partial hospitalization, community, and school-based). Treatment Plan Reviews: Regularly reviews and updates treatment plans based on patient needs and program guidelines.
