Job Description


Job Summary

Serve as a key driver and catalyst for enterprise level population health strategy, clinical program design and evaluation, and monitoring to achieve the Quadruple Aim and drive Molina Healthcare’s Population Health mission. Design and deploy the clinical strategy that supports Molina Healthcare’s comprehensive Population Health program to support and deliver on Molina’s vision, values, goals, operations, and financial performance. Collaborates on all aspects of the strategic plan that focuses on member equity inclusive of major health care concerns and social determinants of health, including but not limited to food insecurities, housing, employment, and incarceration repatriation. Understand the unique community health needs and the attributes of the populations served to drive development of programs and services.

Job Duties

Leads a team of clinical program managers whose responsibility is to design and support deployment of Population Health clinical programs to include organizing and managing   enterprise -wide cross functional workgroups, programmatic content design, dashboard development, clinical and quality outcome metrics, and implementation.Leverages relationships with internal partners to understand needs and respond with scorecards, tools, and processes to drive implementation and evaluation of the Population Health clinical programs.Partners with Clinical Quality Performance Management leadership fostering collaboration and knowledge sharing that aligns strategy while improving member experience.Leverages data analytics, population health knowledge, and initiative implementation skills to drive the development of interventions to target unique populations.Works effectively with clinical and operational stakeholders to synthesize program information, develop and deploy measurement tools and oversee timelines, budgets, and ROI analysis.Oversee all related meetings and committees, directs the data analysis and reporting, monitors program performance and looks ahead to mitigate risks.Creates culture and linguistic program descriptions following NCQA accreditation standards.Ensures documentation of all clinical program design for population health clinical programs.Liaises with legal and compliance in reviewing and evaluating regulatory requirements as it relates to clinical program design.Actively supports clinical program implementation and serves as a resource to MHI training and policy teams.Oversees and navigates technology and software applications utilized in the delivery of population health programs.Responsible for developing effective partnership and engagement strategies with internal and external stakeholders and oversee pilot programs to test and deploy innovative models for Population Health across Molina Healthcare.Contributes to Molina RFP process as requested.

Job Qualifications

REQUIRED EDUCATION:

Bachelor's degree in Nursing, or other health related degree.

REQUIRED EXPERIENCE/KNOWLEDGE, SKILLS & ABILITIES:

5+ years in Managed care, preferably serving members in government sponsored programs.  3+ years' experience in clinical program design.3 + years of progressive experience in population health program design, service coordination, long-term care, community public health, or case or care management.5+ years of progressive people leadership experience to include experience fostering collaboration, teamwork, customer and quality centric focus and employee engagement.Development and execution of evidence-based population health management integration programs.Experience with data analytics and implementing data driven programs and solutions.Population health knowledge and initiative implementation skills.             

REQUIRED LICENSE, CERTIFICATION, ASSOCIATION:

Active, unrestricted State Registered Nursing (RN) OR a Licensed Clinical Social Worker (LCSW) in good standing

PREFERRED EDUCATION:

Master's degree preferred

PREFERRED LICENSE, CERTIFICATION, ASSOCIATION:

CCM Certification, Six Sigma, Project Management certification

PHYSICAL DEMANDS:

Working environment is generally favorable and lighting and temperature are adequate. Work is generally performed in an office environment in which there is only minimal exposure to unpleasant and/or hazardous working conditions. Pay Range: $122,430.44 - $238,739.35 / ANNUAL
