Assistant Social Worker

Essential Functions + Collaborate with an interdisciplinary team—including nurses, aides, therapists, and physicians—to provide coordinated, high-quality care. + Perform initial, quarterly, and annual biopsychosocial assessments for each resident and ensure care plans are accurate and up to date. + Lead care plan meetings for short-term residents with their care team and designated representatives to support personalized care. + Maintain accurate, timely documentation in medical records per facility policies and state/federal guidelines. + Conduct routine chart audits to ensure compliance, accuracy, and regulatory standards are met. + Review admission screens to assess resident appropriateness prior to intake. + Implement and communicate social services policies in line with best practices and regulatory requirements; monitor staff for adherence. + Build relationships with community organizations to promote the facility, strengthen referral networks, and support residents’ transitions back to the community. + Participate in management and discharge planning meetings to address resident needs, census updates, and quality-of-care goals. + Ensure delivery of compassionate, person-centered care, supported by resident feedback, documentation, and observation. + Communicate clearly and respectfully with residents and their representatives, addressing questions and concerns promptly. + Partner with physicians, consultants, and external agencies to enhance services and resolve issues affecting resident care. + Coordinate with other departments to ensure seamless delivery of care and services. + Foster a positive work culture by encouraging teamwork, mutual respect, and effective communication. + Actively participate in the Quality Assessment and Assurance (QAA) Committee and support ongoing QAA activities for social work services. + Monitor compliance through rounds, interviews, and record reviews; summarize findings for QAA and Social Services leadership. + Promote customer service by addressing complaints or concerns from residents and families professionally and promptly. + Monitor incidents and unusual occurrences daily; report significant events to the Director of Social Services as required. + Represent the facility professionally in interactions with residents, families, staff, vendors, and regulatory agencies.
