We are so glad you are interested in joining Sutter Health! **Organization:** PAMF-Palo Alto Medical Foundation ALD **Position Overview:** The Assistant Case Manager 3 works in collaboration with the Physician, Psychotherapist, consulting Psychiatrist, and other care team members to assure the appropriate level of care and coordination for outpatient services. The Assistant Case Manager reports to the Healthy Young Minds Clinical Manager. The Assistant Case Manager has frequent contact with patients, families, physicians, the interdisciplinary team, schools, ancillary services, and community resources.
