Registered Nurse Assessment Coordinator

Type: Full-Time, Permanent

Overview:

Seeking a Registered Nurse to conduct in-home and virtual assessments for health plan eligibility and care planning. This role supports member independence and long-term health through person-centered service plans.

Key Responsibilities:

Perform UAS-NY assessments and evaluate clinical, behavioral, and social needsDevelop care plans and educate members on benefits and servicesCoordinate with care teams and maintain accurate documentationEnsure compliance with DOH and MLTC regulations

Qualifications:

RN licensed in NYS; Associate degree required, BSN preferred1+ year nursing experience preferredMUST have experience with UAS-NY and long-term care Strong communication and EMR skillsValid NYS driver’s license and reliable transportation to conduct site visits

Pay and Benefits

The pay range for this position is $90000.00 - $100000.00/yr.
