• Seeking part-time Licensed APP to support the development and expansion of our Dementia Program
• Minimum of 1–2 years APP experience preferred
• Experience with geriatric or memory care populations strongly preferred
• Outpatient-based care
• No call responsibilities
• Travel required to patient homes within the Flathead Valley, as needed
• Some telehealth services may be incorporated
• Collaborative team approach – working closely with a Nurse Navigator and Medical Assistant
