We have an exciting opportunity for an Alcohol Assertive Outreach Worker who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs. 

 

The role is to outreach service users who have been identified through the assertive outreach model, within their own accommodation. The role will be required to carry a small caseload complex needs service users with a focus on improving health outcomes, alcohol related harms and hospital admissions.  

 

Where: Hull- the role involves travelling to Service Users homes. 

Hours: 37.5 per weeks

Salary: 27,861.26 - 32,002.35 (based on full time hours, pro rata for part time) 

 

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

Responsibilities

About the role: 

 

Work within the scope of the assertive outreach model to positively engage those identified using an assertive outreach approach.Working within an outreach framework model to complete home visits as per the lone working policy to positively engage service users as identified.Complete personalised assessments and Full Risk Reviews.To contribute towards a service users plan.To work within an MDT approach with relevant partners to tailor interventions.To ensure all case notes are accurate and updated in line with policy time-frames.To carry a caseload of up to 15 service users for a period of 12 months.Manage discharge and drop out, in line with criteria as defined by the alcohol assertive outreach provision.

 

 

About you: 

 

Excellent communication skills.Non-judgmental attitude and experience of working with service users.Understanding of drug and alcohol issues. Ability to maintain confidentiality and act within professional boundaries
