We are seeking a compassionate and experienced Registered Nurse (RN) to join our Advanced Illness Management (AIM) Palliative Care Team. This is a field-based position that provides in-person and telephonic care coordination, education, and assessments to patients with serious or life-limiting illnesses.
The AIM Palliative RN will collaborate with an interdisciplinary team including Nurse Practitioners, Social Workers, and Primary Care Providers. This role ensures high-quality, patient-centered care and supports comfort, dignity, and quality of life for patients in private homes, assisted living communities, and skilled nursing facilities.
Key Responsibilities:Conduct in-home and telephonic nursing assessments and screenings for palliative care patientsComplete risk stratification assessments and determine frequency of visits based on patient needsDeliver patient and family education on disease progression, medications, nutrition, and end-of-life careCollaborate with NPs and PCPs to identify changes in patient condition and escalate care appropriatelyCoordinate care transitions and facilitate hospice referrals when neededAccurately document clinical findings, assessments, care plans, and education in the Electronic Medical Record (EMR)Support the development and regular updates of individualized Palliative Care PlansCommunicate lab results, order DME, and manage prescriptions in accordance with state RN scope of practiceParticipate in rotating on-call coverage for evenings and weekendsFoster strong communication and collaboration across the care team, including social work and case managementEnsure adherence to all HIPAA, compliance, ethical, and professional standards About You Required Qualifications:Active and unrestricted Registered Nurse (RN) license with no board actionsMinimum 2 years of clinical RN experience, preferably in skilled nursing, home health, palliative care, hospice, geriatrics, or oncologyDemonstrated ability to provide quality care in high-empathy environmentsProficiency in Microsoft Outlook, Word, Excel, and electronic medical recordsExceptional communication, time management, and critical thinking skillsKeen observation skills with the ability to identify changes in condition and escalate appropriatelyComfortable working independently in the field and within interdisciplinary care modelsValid driver’s license and reliable transportation (field-based role)
Preferred Experience:Previous experience in palliative care, hospice, or oncology nursingFamiliarity with risk stratification, symptom management, and home-based care planningExperience working in or with assisted living and skilled nursing facilities We Offer
