We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as an AIM Clinical Coordinator. This position plays a vital role in supporting the Advanced Illness Management (AIM) Palliative Care program.

 

You’ll be responsible for managing day-to-day clinical operations, scheduling, and care coordination, ensuring seamless delivery of high-quality, patient-centered care.

 

This role reports directly to the Clinical Manager, and serves as a clinical and operational bridge between patients, providers, and our healthcare partners.

 Key Responsibilities:Coordinate and manage all daily clinical activities for the AIM and Palliative Care departmentsSchedule and ensure visit frequency adherence, including PRN visits and post-hospitalization follow-upsServe as the primary point of contact for patient and partner inquiriesProcess patient referrals, ensuring accurate insurance and admission documentationOrder and track labs, imaging, and DME; ensure timely result delivery and documentation in EMRSupport Nurse Practitioners by entering communications and medication reconciliation in partner EMRsIdentify Home Health and Hospice-eligible patients and coordinate timely referralsMaintain and organize clinical records in accordance with federal and state regulationsTrack and manage ED visits and hospitalizations; schedule post-discharge visitsCoordinate interdisciplinary team (IDT) meetings and maintain agendasSupport credentialing processes by submitting necessary documents and maintaining logsPartner with local labs for specimen drop-off coordinationEngage in quality assurance, performance improvement, and marketing initiatives About You Registered Nurse (RN) license in good standingBachelor’s degree in Nursing, Healthcare Administration, or a related field preferred3+ years of experience in healthcare delivery, care coordination, or palliative/home health settingsExperience with Part B billing and knowledge of insurance reimbursement processes preferredProficiency in Electronic Medical Records (EMR) and Microsoft Office tools (Outlook, Word, Excel)Strong understanding of medical terminology, risk scoring, and care transition best practicesExcellent written, verbal, and interpersonal communication skillsDetail-oriented with the ability to manage multiple tasks and shifting priorities We Offer
