We’re looking for a compassionate and skilled Afterhours Registered Nurse (RN) to join our Advanced Illness Management (AIM) team. In this vital palliative care role, you’ll provide after hours clinical support to patients and families facing serious illness—through in-person visits, phone triage, and virtual interventions.
Work schedule is seven days on, seven days off. 5pm-8am during the week, and 5pm Friday through 8am Monday.

Key Responsibilities
Triage and respond to urgent calls from palliative care patients and families after hoursConduct initial nursing evaluations to determine eligibility for palliative care servicesProvide clinical support via phone, video, or in-person visits as neededCollaborate with physicians, nurse practitioners, and the interdisciplinary team (IDT)Assess and manage patients’ physical, emotional, psychosocial, and spiritual needsDocument care accurately and timely in the electronic medical record (EMR)Communicate changes in condition to the appropriate care team, including PCPs and supervisorsAssist with medication, DME, and supply needs in accordance with care plansEnsure continuity of care and contribute to advanced illness care planningAuditing patient charts as needed About YouRegistered Nurse / RN Requirements:
Education:
Associate or bachelor’s degree in Nursing
Experience:
2 years of prior skilled Registered Nurse experienceDemonstrated ability to provide quality patient careNursing experience in palliative care, hospice, home health, oncology, or geriatric care preferredPrior experience in triage nursing, emergency care, or crisis response is a plusAdditional, preferred experience to help you be successful role: hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry or travel nursing
Licenses, Certifications and/or Registration:
Active and unrestrictive RN license to practice in the state without any board actionsCPR Certification
Equipment/Tools/Work-Aids:
Familiarity with electronic medical records (EMR) and Microsoft Office
Specialized Knowledge and Skills:
