Are you living your life's work? Abbeyville Family Medicine, located in Lancaster, Pennsylvania, is searching for a full-time Advanced Practice Provider (CRNP/PA-C) to join our team! The Abbeyville team is currently comprised of 9 Physicians and 3 Advanced Practice Providers. Our collaborative care team works to help patients establish goals, manage their health, connect to community/financial resources, and assists with referrals to other services. Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties: + Consults with patients to determine the appropriate course of treatment. + Recommends and orders tests to determine extent of illness or help diagnose condition. + Monitors lab results, medication adherence and medication toxicities. + Prescribe or administer treatment, therapy, medication, vaccinations, and other medical care to treat or prevent illness, disease, or injury. + Explain and discuss procedures, test results, or prescribed treatment with the patient. + Coordinate work with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers. + Refers patients to medical specialists when necessary. Minimum Qualifications: + Current Licensure as a Physician Assistant or Nurse Practitioner (FNP) + Valid, unrestricted Federal Drug Enforcement Agency (DEA) number + Participation as a provider in good standing in the Medicare, Medicaid, and Tricare programs and such other third-party-payment programs, including managed care programs in white LGMG participates.
