Job Overview:
The Nurse Practitioner will work one day per week seeing patients in-office at Roselle Park Medical Associates, with the potential for afternoon home visits as needed for established homebound patients within the area.
The remaining four days will be spent providing care in a sub-acute nursing home setting.
The position includes on-call responsibilities every fifth week, covering after-hours of 5:00 PM to 9:00 AM, Monday through Friday, and all day on weekends.

Performs comprehensive initial and episodic health assessments; in order to identify existing or potential health problems.Initiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary team.Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician.Provides follow-up care including acute and routine examinations. Assesses progress, identify problems, initiate preventive measures, recognize emergent problems, initiate proper care in collaboration with physician.Monitors and adjusts patients medication according to protocol or medical plan of care. Implements nutritional therapies, as appropriate.Triages patient problem calls which requires medical attention.Orders appropriate laboratory and diagnostic procedures. Recognizes normal and abnormal laboratory findings and the pathology involved.Documents information in patients charts following documentation standardProvides health education to the patient and family (e.g., management of side effects, anticipatory teaching.)Manages medical emergencies as indicated, and according to standing orders.Provides monitoring and continuity of care between health care settings.Other Duties:
