The advanced therapies may include, but are not limited to, organ transplantation and medical therapy. The Clinical Coordinator identifies patients at risk for suboptimal outcomes and proposes a proactive plan of care. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. **The value you'll bring to the team:** + Supports complex patient care issues, identifies patients at risk for suboptimal outcomes and proposes a proactive plan of care. Coordinates and/or supports patient education programs and evaluates impact of patient education programs. + Monitors practice standards and evaluates impact on patient care with identification of problematic systems/process issues. Assists with plans development to improve and monitor outcomes. + Monitors and evaluates nursing practice with identification of professional growth needs and utilizes. Knowledge about current research and evidence-based practices in area of expertise and recommends changes in practice as appropriate. + Acts as a liaison with the Hepatology patient population, the Liver Transplant Coordinator, and community care providers.
