Position Summary:
The Admissions Nurse plays a critical role in coordinating and overseeing the admission process for new resides entering the skilled nursing facility. This nurse ensures a smooth, timely, and compassionate transition for residents and their families while verifying clinical appropriateness, gathering accurate health data, and working collaboratively with interdisciplinary teams.
Key Responsibilities
Admission Coordination
Receive and review referrals from hospitals, physicians, case managers, and other sources.
Conduct clinical pre-admission screenings to access potential residents' needs and ensure appropriate placement
Coordinate with the IDT team members including (nursing, therapy, social services, dietary, etc.) to prepare for the resident's arrival.
Qualifications
Education: Graduate of an accredited nursing program.
Licensure/Certification:
Current RN license in the state of practice
Skills:
Strong clinical assessment and communication skills
Ability to multitask and work in a fast-paced environment
Knowledge of state and federal regulations
Experience with electronic medical records (PCC) systems preferred.
