LifeBridge Health is seeking a dedicated and detail-oriented Admissions Coordinator to join our Post Acute Facilities team. This role is critical in coordinating all aspects of patient admissions, ensuring smooth transitions and an excellent experience for patients, families, and internal staff. If you have a background in LTACH or LTC settings and enjoy working in a collaborative healthcare environment, we want to hear from you.

Key Responsibilities

Coordinate all patient/resident admissions to LifeBridge Health Post Acute Facilities.

Ensure referrals are promptly evaluated for medical and financial eligibility.

Verify insurance coverage and obtain authorizations for LTACH and LTC admissions.

Facilitate the collection and transfer of clinical documentation.

Collaborate with Liaisons, Unit Staff, Clinical Leaders, Physicians, and Patient Accounting.

Conduct family meetings and facility tours (as available).

Maintain accurate bed management and track admission status updates.

Document all admissions in the electronic medical record system (EMR).

Perform registration audits and respond to compliance reviews as needed.

Complete admission paperwork and gather necessary demographic and consent information.

Support clerical tasks and maintain census/admission lists.

Represent and uphold LifeBridge Health’s core values at all times.

Required Experience

1–3 years of experience in a healthcare admissions role

Prior experience in LTACH (Long-Term Acute Care Hospital) or LTC (Long-Term Care)

Preferred Education

High School Diploma or GED (required)

Additional training or certification in healthcare administration (preferred but not required)

Skills & Competencies

Strong knowledge of EMR systems

Ability to communicate effectively with patients, families, and clinical staff

High attention to detail and organizational skills

Understanding of insurance verification and authorization processes

Compassionate and patient-focused approach
