Key Responsibilities:
Serve as the primary point of contact for new patient referralsInitiate outreach to patients and families within 1 hour of referralCoordinate all admissions within the company’s 3-hour commitment windowVerify insurance benefits, payer source, and secure authorizations for hospice careCommunicate pending admissions to appropriate clinical and admission staff.Collaborate with clinical teams, billing coordinators, and external referral sourcesEnter accurate patient data and documentation into the electronic systemParticipate in morning stand-up meetings, marketing activities, and outreach including seeking new referrals sources and developing strategies for company growthPrepare marketing reports regarding referral statistics.Maintain communication with pending and Not Taken Under Care (NTUC) patientsSupport weekend and on-call admissions as needed About You High school diploma or equivalent requiredLPN/LVN license or bachelor’s degree in nursing, Business, Psychology, or Social Work strongly preferredPrevious experience in customer serviceHospice or medical office experience preferredStrong skills in communication, organization, negotiation, public relations and data entryFamiliarity with Microsoft Office and healthcare systemsKnowledge of hospice admission regulations, compliance, and insurance processes is a plus We OfferBenefits for All Hospice Associates (Full-Time & Per Diem):
