**Key Responsibilities:**
+ Serve as the primary point of contact for new patient referrals
+ Initiate outreach to patients and families within 1 hour of referral
+ Coordinate all admissions within the company’s 3-hour commitment window
+ Verify insurance benefits, payer source, and secure authorizations for hospice care
+ Communicate pending admissions to appropriate clinical and admission staff.
+ Collaborate with clinical teams, billing coordinators, and external referral sources
+ Enter accurate patient data and documentation into the electronic system
+ Participate in morning stand-up meetings, marketing activities, and outreach including seeking new referrals sources and developing strategies for company growth
+ Prepare marketing reports regarding referral statistics.
+ Maintain communication with pending and Not Taken Under Care (NTUC) patients
+ Support weekend and on-call admissions as needed
**About You**
+ High school diploma or equivalent required
+ LPN/LVN license or bachelor’s degree in nursing, Business, Psychology, or Social Work strongly preferred
+ Previous experience in customer service
+ Hospice or medical office experience preferred
+ Strong skills in communication, organization, negotiation, public relations and data entry
+ Familiarity with Microsoft Office and healthcare systems
+ Knowledge of hospice admission regulations, compliance, and insurance processes is a plus
