Morton Plant Hospital is on a continuous path of transformation to meet the future health care needs of the community **"Unlock Your Career Potential: Join our Team and Enjoy our Outstanding Benefits!"** + Benefits (Health, Dental, Vision) + Paid time off + Tuition reimbursement + 401k match and additional yearly contribution + Yearly performance appraisals and team award bonus + Community discounts + **AND the chance to be part of an amazing team and a great place to work!** **Acute Rehab Coordinator Responsibilities:** + Lead and supervise the acute rehab team, including physical therapists, occupational therapists, speech-language pathologists, and rehab techs, within the hospital’s acute care unit. + Coordinate daily team operations, managing patient assignments and therapy schedules in alignment with hospital priorities and patient acuity. + Collaborate closely rehab leadership, case managers, discharge planners, nursing leadership, and ancillary departments to optimize patient care plans and ensure timely discharge. + Provide clinical leadership and mentorship to rehab staff, fostering professional growth and maintaining high standards of care. + Monitor patient progress and rehab outcomes, ensuring compliance with hospital policies, Joint Commission standards, and regulatory requirements. + Lead team meetings to review patient cases, address challenges, and streamline workflow in the hospital setting. + Ensure accurate, timely documentation of rehab services within the hospital’s electronic medical record (EMR) system. + Participate in hospital-wide quality improvement initiatives related to acute rehabilitation services. + Assist with staff recruitment, onboarding, and scheduling to maintain adequate coverage in the rehab department. + Address patient and family concerns, promoting a patient- and family-centered care approach within the hospital. + Maintain strong communication channels among multidisciplinary hospital teams to support coordinated care transitions. **Requirements:** + Bachelors – Physical Therapy, Occupational Therapy, Speech Therapy Or Bachelors – Related Field + BLS (Basic Life Support) + Licensed PT (Physical Therapy), OT (Occupational Therapy), Or Speech Therapy (CCC-SLP) **Facility:** BayCare Health System, Rehab Services Main-MPH **Location:** Morton Plant Hospital, Clearwater, FL **Status:** Full Time, Exempt: Yes **Shift Hours:** Monday - Friday 7:00 a.m. to 5:00 p.m./ Varies **Weekend Work:** Occasional **On Call:** Yes (Via text or call) **How often will this team member be working remotely?** Never Equal Opportunity Employer Veterans/Disabled **Position** Acute Rehab Coordinator **Location** Clearwater:Morton Plant | Clinical | Full Time **Req ID** null
